DBE Application - complete if your firm is seeking federal contracting opportunities with Federal Highway Administration, Federal Aviation Administration, & Federal Transit Administration.
Annual Update (DBE) - annual renewal form for all DBE & ACDBE certified firms.
MBE/WBE Application - complete if your firm is ONLY seeking local contracting opportunities. This application alone will not qualify your firm as a DBE.
Annual Update (M/WBE) - annual renewal form for all MBE & WBE certified firms.
PNWS Application - personal net worth statement required for all DBE & ACDBE applicants. This document is already included in the TUCP-DBE application and can be used for multiple owners.

ITEMS BELOW ARE REQUIRED FOR ALL ACDBE & DBE RENEWAL APPLICANTS!

 

All DBE certified firms MUST submit the following in addition to the documents according to business structure: 

Complete copy of 1040 Individual Tax Return (all pages/schedules) & W2 forms.

Sole Proprietor:

(1) Renewal form; (2) Complete copy of 1040 Tax Return (including Schedule C); (3) Any licenses required to operate business.

 

 

Partnership:

(1) Renewal form; (2) Complete copy of 1065 Tax Return; (3) Any licenses required to operate business.

 

 

Corporation:

(1) Renewal form; (2) Complete copy of 1120/1120S Tax Return; (3) Minutes of last annual Meeting; (4) Any licenses required to operate business.

 

 

LLC:

(1) Renewal form; (2) Complete copy of Business Tax Return; (3) Minutes of last annual Meeting (if applicable); (4) Any licenses required to operate business.

Sole Proprietor:

(1) Renewal form; (2) Schedule C of 1040 Tax Return (Only M/WBE); (3) Any licenses required to operate business.

 

Partnership:

(1) Renewal form; (2) Complete copy of 1065 Tax Return; (3) Any licenses required to operate business.

 

Corporation:

(1) Renewal form; (2) Complete copy of 1120/1120S Tax Return; (3) Minutes of last annual Meeting; (4) Any licenses required to operate business.
